Position Title: Supportive Housing PSW
Department: Supportive Housing Adult Day Program


PURPOSE: To provide a variety of non-medical support/care services to the vulnerable population (individuals and families) in their place of residence REPORTS TO: Program Manger, Supportive Housing Department POSITION DESCRIPTION: 1. To provide in-home services to frail elderly clients, to maintain one’s safety and independence in their own home for as long as possible, through the provision of assistance with their activities of daily living, respecting their ethnic and culturally specific needs, in order to prevent premature nursing home placement. Activities – encouraging client independence, through client-specific motivational techniques and through active socialization. – providing instruction on activities of daily living, via demonstrations, according to the Client Care Plan. – providing personal care as required, including: bathing, grooming, dressing, toileting, feeding, transfers/mobility. (PSWs only) – performing a variety of household duties such as light housekeeping, laundry, shopping, to maintain a hygienic environment. – planning and preparing nutritious meals including special diets as required by the Client Care Plan (specific instructions developed for each client), as required. – accompanying/escorting to medical appointments, as required, for support and/or translation assistance. 2. To observe and report any changes or problems related to the client or client’s environment to the Program Manager and/or designate. Activities – observations may be reported wither verbally and/or in writing, in a timely and efficient manner (i.e. ‘incident reports’, time sheets). 3. To attend/participate in Orientation, Training and In-service Courses. 4. To provide client services in accordance with an established time schedule, manner and skill required, as per the client’s Care Plan, while adhering to Departmental Quality and Risk Management principles, ensuring the provision of quality service and safety precautions to clientele. 5. To maintain the confidentiality of information pertaining to clients and families. KNOWLEDGE: 1. A thorough knowledge of homemaking skills acquired through attendance at an orientation program and/or related experience and successful completion of a basic on-site training course. 2. Awareness of the emotional, cultural and social needs of clients. (The ability to deal effectively with clients will vary according to the Homemaker’s previous life experiences and formal education.) 3. Additional skills acquired through in-service training or via direct instruction from an appropriate professional and/or institution. 4. Ability and commitment to be trained to Personal Support Worker Level II/III, as necessary. JUDGEMENT: Sound judgment is required to organize and prioritize daily tasks using initiative, discretion and adaptability, awhile being sensitive to each client’s demands and expectations and adhering to Departmental policies and procedures. CONTACTS: Internal -Regular contact with the Program Manager or designate for direction and information -with other Home Support & Personal Support staff at team-meetings and In-Services. External -ongoing contact with clients, family members and healthcare professionals. (HSW/PSW must be able to relate to these individuals to provide quality clients care to clients through effective communication and coordination). Contact with HSWs/PSWs from other agencies while participating in Personal Support Worker Level II, III and other specialty training courses.
